Initial consultation
Assessment
Intervention

  • The Initial Consultation is a crucial appointment for the primary caregiver and/or client. Its purpose is to lay the groundwork for effective therapy planning by ensuring a comprehensive understanding and preparation before therapy or assessment begins. Notably, the client’s presence is not required for this session.

    Key Components
During the consultation, the primary caregiver and therapist discuss the NDIS plan, goals, and therapy needs to determine the most suitable services. Information about our team, processes, and approach is also provided.

    Intake Paperwork Review
The therapist and caregiver review and confirm completion of all necessary intake paperwork, including consent forms and medical history.

    Conclusion
Following the consultation, the therapist will recommend the necessary assessment. If you've recently had one, the therapist may be able to use it to proceed directly to intervention.

    Duration
Appointments are typically scheduled for 45 minutes, with an additional 15 minutes allocated for clinical documentation. However, if the full session time is not required, our clinicians may adjust the appointment to 30 minutes, ensuring that care remains responsive and tailored to the client's needs.

  • After the initial consultation your therapist will arrange the suitable assessment for you. In this session, they'll employ various techniques to gather crucial information for diagnosis, treatment planning, goal setting, and rapport-building, ensuring a comprehensive understanding and delivering a detailed report.

    Duration
    Comprehensive Assessment (Most Common)
The AHA Comprehensive Assessment typically includes a 2-hour appointment, followed by 3 hours of report writing. This allows for an in-depth understanding of the client's needs. In some cases, a different type of assessment may be more appropriate—your clinician will guide you through the best option based on individual requirements.

    Standard Assessment
The Standard Assessment includes a 1-hour appointment with 1.5 hours allocated for report writing.

Assessment types are determined by the clinician based on the client's goals, needs, and presenting concerns.

  • Allied health intervention sessions are a dynamic collaboration between the therapist and client, tailored to meet individual needs for holistic well-being and functional improvement. During ongoing therapy, personalized techniques and strategies are employed to achieve specific goals identified during the assessment. An Individualised Care Plan (ICP) is created and regularly reviewed to effectively monitor progress throughout the intervention.

    Each session typically includes:

    • 15 minutes for session planning

    • 45 minutes for the intervention session

    • 15 minutes for note-taking

    • 15 minutes for case conferencing, if necessary

This structure ensures ample time for high-quality care and thorough communication with the client’s care team.

High quality, evidence based and tailored therapy services.

FAQ

  • The Individualised Care Plan (ICP) is a collaborative document meticulously crafted by our clinicians at Glasswing Therapy, working in partnership with clients, their families, and caregivers. Its primary objective is to act as a guiding tool, facilitating the monitoring of goals and progress throughout each therapy block. The ICP guarantees that our dedicated clinicians are not only working diligently but are also achieving objectives that revolve around the client and their family.

  • The development of an ICP is an essential requirement for all our clients and therapy blocks. This compulsory procedure not only maintains a steadfast level of therapy excellence but also supports both clinicians and clients with a resilient framework to cultivate goals that are not only significant but also practical.

  • The Individualised Care Plan (ICP) process begins during your initial consultation. Your clinician will gather information to start drafting the ICP and complete during the next scheduled visit where your therapist can take the time to discuss with you and make any necessary adjustments before the finalisation of the ICP occurs. These plans are then reviewed quarterly or at the end of a therapy block and new information can be provided to your therapist either in person or via Telehealth to update the ICP.
